Spring is upon us. Now that the weather is improving, I'm

Diabetic Men More Prone to Urinary Woes
NEW YORK (Reuters Health) - Men with diabetes are at
increased risk for bothersome lower urinary tract
symptoms and frequent nighttime urination or "nocturia,"
investigators have shown. This is particularly true for
black men with diabetes.
The investigators examined the associations between
diabetes and signs of an enlarged prostate gland, a
condition called benign prostatic hypertrophy, or BPH,
that is quite common among older men and that can cause
symptoms such as frequent urination and a weak urine
stream.
The study was made up of 2484 black and white men who
were participating in studies designed to evaluate the
natural history of BPH. Their average age was 56 years,
and 170 (6.8 percent) had diabetes. None had prostate
cancer.
Dr. Aruna V. Sarma, at the University of Michigan in Ann
Arbor, and colleagues found that diabetes significantly
increased the risk of irritative symptoms such as frequent
urination -- particularly at night -- and feelings of
urgency.
The association between diabetes and moderate-to-severe
lower urinary tract symptoms was greater among black men
than among white men.
There were no significant associations between diabetes
and prostate volume, suggesting, the researchers say, that
the "presence of diabetes may be less related to prostate
growth and more related to the dynamic components of lower
urinary tract function."

Heart Patients with Diabetes Need Extra Care
NEW YORK (Reuters Health) - People hospitalized because of
acute heart failure face an increased risk of dying in the
hospital and in the longer term if they have diabetes or
pre-diabetes, researchers report.
The finding "could help target some patients for more
intensive therapy," write Dr. John J. V. McMurray from
the University of Glasgow, UK, and colleagues in the
medical journal Heart.
The team studied 454 consecutive patients admitted to one
university hospital for heart failure; 110 of them (24
percent) had diabetes, 60 (13 percent) had pre-diabetes
indicated by high blood levels of glucose, and 284 had
normal blood glucose.
Fifty-one patients (11 percent) died in the hospital, and
according to the investigators, there was a "striking
relationship between random blood glucose concentration
on admission and total mortality," even after taking into
consideration numerous factors that could increase the
odds of dying.
Death rates in the hospital and later on "were higher in
patients with abnormal glucose tolerance or diabetes,"
they report. "Even mild elevations in blood glucose
predicted mortality."
"Heart failure patients with abnormal glucose tolerance
should receive intensive medical management in order to
improve prognosis," McMurray and his colleagues conclude.

Diabetic Recipe
Sour Cream Chocolate Cake
(makes 16 squares)
baking spray with flour
1 cup (140 g) unbleached all-purpose flour
1/2 cup (70 g) one-to-sugar substitute
1/4 cup (35 g) nonfat dry milk powder
1/2 teaspoon (2.5 ml) baking soda
1/4 teaspoon (1.25 ml) salt
2 tablespoons (25 g) margarine
1 1/2 ounces (45 g) unsweetened baking chocolate
1/2 cup (120 ml) low-fat sour cream
2 tablespoons (60 ml) liquid egg substitute
1/2 teaspoon (2.5 ml) vanilla extract
2 tablespoons (30 ml) strong coffee
1. Preheat oven to 350°F (180°C), Gas Mark 4. Coat an
8-inch (20 cm) square baking pan with the baking spray.
2. Sift the flour into a bowl. Stir in the sugar
substitute, dry milk, baking soda and salt.
3. Melt the margarine and chocolate in the microwave or
in the top of a double boiler. Beat in the sour cream,
egg substitute, vanilla, and coffee. Stir the chocolate
mixture into the dry ingredients. Blend well.
4. Spread into the prepared pan and bake for about 20
minutes until toothpick inserted in the middle of
the cake comes out clean. Cool in pan and cut into
16 squares.
Per serving: 77 calories (35% calories from fat),
2 g protein, 4 g total fat (1.5 g saturated
fat), 12 g carbohydrates, 1 g dietary fiber,
3 mg cholesterol, 110 mg sodium
